We request that you let us know 48 hours in advance if you are unable to keep your scheduled appointment in order to avoid a cancellation fee.  This will help us offer your appointment slot to other patients who may be on the waiting list to be seen.  We are asking for your cooperation in this regard to help serve all our patients adequately and offer appointments with minimal delays in scheduling. 

We accept all major insurances.  Please call the office to verify if your insurance is accepted
For Your Appointment:

If you are seeing us for the first time, please arrive 15-20 minutes prior to your scheduled appointment.

For every appointment, ( first appointment and any subsequent appointment for follow up or sick visit), please bring the following with you:

1. Up-to-date medical insurance information/card
2. All your medications (prescription & OTC) vitamins and supplements
3. List of refills you need, if any
4. Copayment / Coinsurance/Deductible (for your convenience, we accept cash, check or credit cards)
6. Printed registration forms duly filled from the website if this is your first visit

We ask that you come fasting for your appointment for the following reasons:
( This means do not eat for 10-12 hours before your appointment.  Water or black tea or coffee without sugar is permitted ) 

1.  You are an adult coming for an annual physical
( You do not need to fast if you are under 18 years of age or if you are pregnant or if you are coming for a pre-operative physical )

2. You are coming for a follow up on your cholesterol or triglycerides

3. You have been specifically instructed to come fasting at your courtesy appointment reminder call.
